Abstract

Gynecomastia correction remains one of the most common aesthetic procedures performed in men, with many seeking effective outcomes while minimizing visible scarring. This presentation introduces a refined approach that combines VASER-assisted liposuction with minimal incision gland excision, delivering excellent contour and high patient satisfaction.
We present our experience based on a multi-center retrospective analysis spanning from 2009-2025, recently published in Aesthetic Plastic Surgery (Springer, 2025), demonstrating consistent results in patients with moderate to advanced gynecomastia. Aesthetic outcomes were evaluated using clinical photography and subjective satisfaction scores. Most patients achieved improved chest definition, natural contour, and near-invisible scarring due to a lateral areolar incision technique. The technique eliminates the need for peri-areolar skin removal or quilting stitches in most cases. No major complications were observed.
Our experience supports the efficacy of combining VASER with targeted glandular excision to achieve smooth, natural outcomes while limiting scars. In advanced cases where significant skin redundancy remains, a two-staged approach—initial VASER-assisted liposuction with gland excision followed by delayed skin excision 4–6 months later—has proven effective in preserving aesthetics while ensuring contour refinement. This evolving protocol enhances safety and aesthetic quality while tailoring management in more complex cases.
